WHO QUALIFIES?
- Pregnant women.
- Women who are breastfeeding an infant under one year old.
- Women who have had a baby within the past six months.
- Children under the age of five.
In addition, all participants must:
- Live in Utah County
- Meet Income

- Meet certain nutrition criteria (determined at your application appointment).
HOW DO I APPLY?
- Call the WIC clinic nearest you (see WIC Home
Page for locations).
- Make an appointment
- What to Bring:
- Proof of Identification for each applicant.
- Photo identification - such as a driver's license
- Current Medicaid card
- Birth Certificate
- Hospital Card/bracelet (for newborn infant only)
- Pay check stubs issued in your name within the past 30 days
- Military ID
- Passport
- Work or school ID
- Voter registration card
- Letter from human services for foster children
- Letter from Midwife for home births
- Proof of Residency (Address)
- Current bill - such as a utility bill
- Current Medicaid card
- Rent or mortgage receipts for lodging/housing
- Pay check stub with current address listed
- Current bank statement
- Letter from Human Services on official letterhead
- Proof of Income
- All pay stubs from the past month for everyone working in the house
- Signed statement from employer indicating gross cash earnings for
a specified period
- Current Medicaid card
- Income tax return for the most recent calendar year
- Military Pay: recent leave and earnings statement
- Public assistance or welfare payments: Check stub/award letter stating
current amount of earnings
- Alimony and child support: Divorce decree, award letter, copy of
check received, letter from source
- Unemployment compensation: unemployment letter/notice
- All persons applying
Since qualifying for WIC requires collection of health data, such as
weight, height and hemoglobin, we need each person applying for the
program to be present at the appointment. However, exceptions can be
made for people with certain medical conditions. If those apply to you,
print the following referral form to be completed and signed by your
doctor - original signature by doctor is required.
