H1N1 Vaccination General Information

H1N1 Vaccine is now available to any one who wants the vaccine. Vaccine is available in both injectible and nasal spray forms at no cost at UCHD clinics.

H1N1 Vaccine is available on a first-come/first-served basis in our Provo Clinic (151 S. University Ave, click here for hours). Vaccine is also available at our Satellite Clinics in American Fork and Payson.  American Fork and Payson both require appointments, which can be made in advance by calling 801-851-7024 or by clicking "Schedule Now" below.

Walk-in, first-come/first-served mass vaccination clinics are always scheduled in the community. No appointment is necessary, however if you wish to print off an Encounter Sheet to speed the process, you can do so by clicking here (one sheet per person).

There is no cost for the vaccine, however we do ask for insurance information to help recover administrative cost. If you don't have insurance, or we don't take it, you will not receive a bill for an H1N1 vaccine.

Children that have received one dose of the H1N1 vaccine should receive a second dose -- or "booster" -- at least 28 days after the first vaccination. This booster should be obtained as soon as possible after the 28-day waiting period.

H1N1 Mass Vaccination Clinics
The UCHD will also conduct special "H1N1 Mass Vaccination Clinics" in cooperation with community partners. These are open to any member of the pubic, and will offer the H1N1 as well as seasonal influenza vaccines. No appointment is necessary -- individuals will be served on a first-come/first-served basis.

Flu Shots (Seasonal Influenza Vaccination)

Our main office is located in Provo with Satellite Clinics in American Fork and Payson.  The Provo Clinic is a walk-in clinic based on first-come/first served. American Fork and Payson both require appointments, which can be made in advance by calling 801-851-7024.

We accept select health insurances, Medicaid and Medicare for flu vaccines (click here for the complete list).  For those who are uninsured, underinsured, and have Medicaid, the Vaccines for Children (VFC) Program provides vaccines at a low-cost. Find out if you qualify for the VFC Program.

You must get both a SEASONAL flu shot
as well as an H1N1.
You will not be protected without both vaccinations.
