Wallet-size birth cards:
As of August 1, 2006 Utah's wallet-size birth card is no longer available and is not acceptable for legal purposes. If the card is all you have it will need to be replaced with a new paper birth certificate, cost is $18. Certified copies issued on the rainbow paper prior to August 1, 2006 remain valid.
Working with the State Office of Vital Records and Statistics, we are here to provide you with certified copies of birth and death records. Certificates may be obtained by the individual listed on the record or an immediate family member of the person whose record is being requested. An "immediate family member" is defined as a a spouse, parent, sibling, child, grandparent or grandchild. Birth and Death records are filed according to the state where the event occurred. Therefore, births or deaths occurring in Utah are only available through Utah. If the event was in another state, a certified copy of the record can only be obtained by applying directly to that state. We do not maintain marriage records; they are kept by the county clerk's office. Any questions or requests for further information concerning vital records are welcome. |
