Home Visitation Programs
• Prenatal 0-5 Home Visits
Registered nurses make home visits to infants at risk for delay in development. These risk factors may include: prematurity, low apgar scores, birth trauma, substance abuse, and need for parenting skills.
• Medicaid/Targeted Case Management
A Medicaid benefit for infants and families. Registered nurses make home visits to assess the needs of families with referrals to community agencies.• Teen Parents
Parents under the age of 18 receive home visits from registered nurses. Services provided may include: parenting information, and referrals to community agencies.• Spanish Home Visitation Program
Para-professionals make home visits to teach parenting skills and appropriate developmental milestones.• United Way Welcome Baby
Home visit services are provided by trained volunteers focusing on first time parents.• Spanish speaking home visitors are available
