Registered nurses make home visits to infants at risk for delay in development. These risk factors may include: prematurity, low apgar scores, birth trauma, substance abuse, and need for parenting skills.
• Medicaid/Targeted Case Management
A Medicaid benefit for infants and families. Registered nurses make home visits to assess the needs of families with referrals to community agencies.
• Teen Parents
Parents under the age of 18 receive home visits from registered nurses. Services provided may include: parenting information, and referrals to community agencies.
• Spanish Home Visitation Program
Para-professionals make home visits to teach parenting skills and appropriate developmental milestones.
• United Way Welcome Baby
Home visit services are provided by trained volunteers focusing on first time parents.
• Spanish speaking home visitors are available
