The immigration physical process requires 2 appointments and takes approximately 2 weeks for completion.

Call to schedule your first appointment at 851-7038.
Appointments are only available on Monday’s between 1:00 pm-2:45 pm and Wednesday’s 8:00 am-11:00 am.                 

 The following are required for the first appointment:

• Payment of $100
• Cash or Credit Card only
• Current immunization records
• I-693 form
• Fill in personal information (first page only) and bring to clinic
• Form can be found at www.uscis.gov/files/form/I-693.pdf
• TB screening test and required immunizations are available through the immunization program (additional fee)
• Blood draw included in fee

Second appointment:

• Physical by Joseph K. Miner, M.D.
• Dr Miner is on the list of Civil Surgeons approved by the U.S. Immigration and Naturalization Services
• I-693 form will be completed and place in a sealed envelope for delivery to the USCIS office
