Immigration The immigration physical process requires 2 appointments and takes approximately 2 weeks for completion. The first appointment is Monday - Friday 1:00 pm to 3:00 pm. It is necessary to call and schedule an appointment for this. For this appointment you must bring: a. $80 cash - (this pays for your blood draw & physical only) b. I-693 form - we prefer an original (this can be obtained from the INS or an attorney) c. Current immunization records First Appointment: You will pay your money, fill out your I-693 form, I-693 supplemental form and additional forms needed to complete this process. You will be sent back to immunizations for a blood draw and needed immunizations. The blood draw is included in the $80 fee. Immunizations are an additional fee. At this time you will be given an appointment for your physical, which is usually 2 weeks later. Second Appointment: You will meet with Dr. Miner and you will have your physical. After this appointment your I-693 form will be complete and will be processed and placed in an envelope and given to the person to give to Immigration. Dr. Miner is on the list of doctors who are approved by the U.S. Immigration and Naturalization Services to perform medical examinations.
