Class 4 Food Permit (75+ seats, 5000+ sq. ft.) $220.00
Elementary & Middle School Lunch $110.00
Junior & Senior High School Lunch $170.00

* A 10% penalty will be added to the renewal fee of payments for each 30 days late. Permit revoked after 60 days past due. The owner/operator will need to apply and pay for a new permit. The owner would still be required to pay the late fee, and the establishment also must meet current standards.

Facilities opening without a permit are assessed a 100% penalty fee.

Facilities requiring extra or follow-up inspections due to inspection scores will be assessed a $80.00 inspection fee for each additional inspection.

** Food establishments that open without providing plans will be assessed the plan review fee prior to a pre-opening inspection. This applies only to new or remodeled facilities.

*** $5.00 per year for up to 3 years.

License-Exempt Inspection - $40.00 per inspection
